Summary of Responsibilities: Under the general direction of the Children and Youth Supervisor, this role is responsible in providing case management for Children and Youth services to families. Duties are conducted in accordance with all existing policies, procedures, and regulations and in accord with the Mission and Values of the Company. This position has been designated to be a part of the County's CYS workforce for purposes exclusive to the proper handling of consumer information as is required by SAM privacy policies. Essential Functions: Completes in person & collateral contacts with families Assesses safety and risk of children on assigned caseload Develops individualized Family Service Plans Serves as an advocate for families Responsible for all required documentation Assists individuals/families with referrals to outside services Provides court testimony as needed Work may frequently be after hours, and on-call hours are both part of the position requirements. Personal Characteristics: Independent, Tech-Savvy, Responsive, Kind, Ethical, Motivated, Active Listener, Life-Long Learner, Adaptable, Confidential, Resourceful, Values, & Trustworthy Education and Experience: Bachelor's degree with completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; OR two years of experience as a County Social Services Aide 3 and two years of college level course work which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; OR Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions Employment Requirements: Automobile and Valid Insurance, Valid Pennsylvania Driver's License, acceptable Motor Vehicle Record Check, Act 33 and Act 34 Clearances, FBI Fingerprint Clearance, Social Security Card, Immigration and Naturalization (I-9), Drug Screening Clearance Working Conditions: Prolonged periods working on a computer in an open office environment, comfortable communicating through virtual technology, and travel throughout the assigned region may occur and may be in adverse weather conditions, must be able to lift to 15 pounds.
